International Students

International Applicants irrespective of course or country will apply through the Office of International Student Affairs.

MINIMUM ADMISSION REQUIREMENTS FOR MD:

  • Applicant must be a holder of a baccalaureate degree;
  • Interview upon submission of required documents (for scheduling); and
  • Applicant must have taken the National Medical Admission Test (NMAT) within the validity period and must meet preferred NMAT percentile rank of 40 or better. Applicants who wish to improve their NMAT percentile score may retake the test.

Documents to Submit for MD:

  1. Accomplished Application Form
  2. Original/Certified True Copy of Transcript of Records
  3. Certified copy of AB/BS Diploma/Certificate of Candidacy for Graduation
  4. Original Certificate of Good Moral Character and two (2) letter of recommendation from 2 former professors.
  5. Birth Certificate
  6. National Medical Admission Test (NMAT) Result (VALID NMAT:  October 2019 to 2021)
  7. Marriage Certificate, if married
  8. 2×2 Pictures (2 Pieces Colored with White Background)
  9. 1×1 Pictures (2 Pieces Colored with White Background)

(Item 2-5 authenticated by the External Affairs or Philippine Embassy in the countries where no bilateral agreement on notarized documents exist.)

Student Visa

  1. Original copy of the school’s Notice of Acceptance issued by the Office of the Dean containing a clear impression of the school’s official dry seal and addressed to the student;
  2. Letter from the School Registrar requesting the issuance of a Student Visa 9(f) to the applicant addressed to: The Director, Visa Division – Office of Consular’ Affairs.
  3. Six copies of the original Personal History Statement duly accomplished and signed by the applicant, both in English and his national alphabet, accompanied by personal seal, if any, and containing among others, his/her original left and right thumb prints and a 2″ x 2″ photograph on plain white background taken not more than six months prior to submission;
  4. Transcript of Records/Scholastic Records, duly authenticated by the Philippine Embassy or Consulate in the applicant’s country of origin or legal residence;
  5. Notarized Affidavit of Support, including bank statements or notarized notice of grant for institutional scholars to cover applicant’s expenses for school dues, accommodation, subsistence and other incidental expenses;
  6. Original copy of the CEA approved by the CHED;
  7. Visa Application form (FA form no. 2);
  8. Medical Certificate (FA form no. 11);
  9. Chest x-ray, laboratory report and clearance certificate for the AIDS antibody (HIV test);
  10. Police Clearance from the local police authorities where applicant has been permanently residing for the last five years;
  11. Guarantee coverage of return fare and other incidental expenses;
  12. Life-size chest x-ray film;
  13. Unexpired passport

ADDITIONAL DOCUMENTS FOR INTERNATIONAL STUDENTS:

  • Passport photocopy with relevent pages icluding the page where name, photo, birthday, etc appear.
  • Police Clearance issued by the National Police in the student's country of origin or residence
  • Students Personal History Statement with 2 x 2 recently taken photograph - 6 copies.
  • Notarized Affidavit of Support, including bank statements to cover applicant's expenses for college academic fees, accommodation, subsistence and other expenses;

PLTCI APCMR Office of International Student Affairs endorses the above documents to the Department of Foreign Affairs (DFA) Manila, together with the NOA and CHED-approved CEA for processing. Once approved, the DFA notifies the Foreign Service Post concerned on the issuance of a student visa for 9F.

Student might also opt to apply for 9A in the home country. while the documentary requiremnts are the same, the processing time is significantly shorter. The student will have to convert his/her visa to student visa (9F) after arriving in the Philippines.
